On your initial visit, please bring the following items:

• Picture identification
• Insurance card(s)
• List of medications you are taking
• Pertinent test results, such as CT scans, X-rays, sleep studies, and/or hearing test results​​​
• Patient Forms (Download PDF)

Also, be prepared to share your medical history with our physicians. This will include any medical conditions you have or may have had in the past, surgical history, any medications (prescription or over-the-counter) you are currently taking (include name, dosage, and frequency), and any allergies you have to medications or foods.

Payment is expected when services are rendered. We accept cash, check, Visa, MC, Discover, and AMEX.
